In Saskatchewan, the Ministry of Social Services issues Requisitions for Goods and Services vouchers to needy citizens. Understanding the requisition process is crucial if you’re a vendor looking to participate in this program. Here’s what you need to know:

Requisition for Goods and Services

  • Social workers complete and sign the requisition with the client before issuance.
  • The requisition includes the citizen’s name, address, date issued, description of goods/services, and maximum value.
  • Three copies (white, blue, and green) are provided.

Vendor Instructions To ensure reimbursement:

  • Verify that the purchase matches the description on the requisition and doesn’t exceed the indicated amount.
  • Obtain the client’s signature on the requisition to confirm receipt of goods/services.
  • Submit the white copy with the original sales slip or invoice to the Ministry of Social Services within 30 days.
  • Retain the blue and green copies for your records.

Further Information

  • Requisitions expire after six months from the date of issue.
  • They should never be used for tobacco products unless specified by the worker.
  • Cash transactions with the purchaser are prohibited.
  • Refunds for returned goods must be made to the Ministry of Social Services.
  • Purchases are not subject to GST; any charged GST will be deducted from the reimbursement.
