- Verify that the purchase matches the description on the requisition and doesn’t exceed the indicated amount.
- Obtain the client’s signature on the requisition to confirm receipt of goods/services.
- Submit the white copy with the original sales slip or invoice to the Ministry of Social Services within 30 days.
- Retain the blue and green copies for your records.

- Requisitions expire after six months from the date of issue.
- They should never be used for tobacco products unless specified by the worker.
- Cash transactions with the purchaser are prohibited.
